We offer in person or Telehealth appointments!


TeleHealth Visit

What is a telehealth visit? 

A telehealth visit is the use of digital information and communication technologies, such as computers and mobile devices, to access health care services remotely and manage your health care. In short- it's a video chat with a doctor through your phone, tablet, or computer from the comfort of your home!

How does it work?

  1. You call our office and request an appointment (Psychiatric Evaluation, Medication Management, or Talk Therapy). We will gather your information from you with the best way to reach you for the video chat. You will be asked to email us a picture of your Driver's License and Insurance Card.
  2. You will email us a picture of your Insurance Card (front and back) and your driver's license. 
  3. You will get an email with a link to our patient portal. Once in the patient portal you will fill out the patient documents under intake documents.
  4. You will be asked to pay your copay when you schedule your appointment. If you are unable to do so at that time, you will need to call back and make the payment at least 24hrs prior to the visit. Appointments that have not been paid for are subject to cancellation. 
  5. At the time of your scheduled appointment you will get a text message or email (whichever platform you have designated to do your telehealth visit through) from the provider you are seeing that will have a link attached to it.
  6. You will click the link. 
  7. After clicking the link, a pop up will come up on your device requesting access to your camera. CLICK YES/OK/ALLOW. 
  8. Another screen will come up from doxy.me welcoming you and will ask you to enter your name. Enter your name then click the "Check In" button.
  9. After that you will be connected with the provider and the video visit should connect momentarily.

GUIDELINES FOR TELEHEALTH APPOINTMENTS

*If you have an appointment with Dr. Saleem for potential medication management, please have your current medication bottles within reach prior to the visit starting.

*We ask that you find a quiet place away from distractions to have your visit. 

*Absolutely NO DRIVING during your telehealth visit!


In Office Visits 

Please arrive at least 25 minutes prior to your appointment time to allow sufficient time to complete any necessary paperwork. If you arrive for your appointment more than 15 minutes late, please be aware that your appointment may be rescheduled, as to not inconvenience the next scheduled patient. 

If you cannot keep your appointment, please call 804-897-9355 to cancel or reschedule your appointment AT LEAST TWENTY FOUR (24) BUSINESS HOURS IN ADVANCE. Failure to do so will result in cancellation fees equal to that of a missed appointment.


WHAT TO BRING TO YOUR APPOINTMENT
• Valid Photo ID
• Insurance Cards

• Prescription Cards
• Completed Patient Forms; forms can be printed and brought with you or filled on the patient portal
• List of current prescriptions and/or over-the-counter Medication, including dose and frequency
• Information about your medical history or other relevant information
• Payment in the form of credit card. NO CHECKS.

Insurances Accepted & Payment

Our providers participate with most major insurance plans such as Medicare (ages 65 and up), Tricare, Anthem, Aetna, Cigna, and Optima/Sentara. We also provide a self-pay rate for those who are uninsured. Please call our office at 804-897-9355 and we will gladly answer any questions you have.
