MEDICATION POLICY

Recently we have been receiving a lot of requests from pharmacies for medication refills for medicines that have either been discontinued or the dosage has been readjusted. These requests are often times automatically generated by the pharmacy systems and they pose a risk of medication errors.


Effective  March 1st, 2021 we will no longer accept refill requests from pharmacies by phone, fax or e-mail. The goal as it has always been is to renew all medications during office visits. 

Patients taking long term medications must be seen at regular intervals as advised by Dr. Saleem. All prescriptions will be filled at your appointment with sufficient refills to last until the next scheduled appointment. 


To ensure you do not run out of your medication, it is advised that you schedule your next follow up appointment at the end of each appointment. 

In the event that you run out of your medication and have not scheduled your follow up appointment in time, please call the office, send a message though the portal, or send us an email. We will try our best to get you in as soon as possible. 


It is important to understand this change will enable us to provide quality care as medication renewals outside the office visit require much research by the office staff to ensure the medication request is appropriate/accurate and that dosing is based on the latest information obtained during the previous office visit.
