Please arrive at least 15 minutes prior to your appointment time to allow sufficient time to complete any necessary paperwork. If you arrive for you appointment more than 15 minutes late, please be aware that your appointment may be rescheduled, as to not inconvenience the next scheduled patient. 

If you cannot keep your appointment, please call 804-897-9355 to cancel or reschedule your appointment AT LEAST TWENTY FOUR (24) HOURS IN ADVANCE! Failure to do so will result in cancellation fees equal to that of a missed appointment.

What to Bring to Your First Appointment

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
• Valid Photo ID
• Insurance Card
• Completed Patient Forms; forms can be printed and brought with you or filled on the patient portal
• List of current prescriptions and/or over-the-counter Medication, including dose and frequency
• Information about your medical history or other relevant information
• First time patients must pay by cash or charge (checks are not accepted for initial evaluations)

Payment Info

Our providers do participate in the preferred provider networks for Cigna, Anthem, and Tricare. We do provide services for "Fee for Service". Payment for all services is, therefore, required at the time of visit. Depending on your health insurance plan you may have out of network benefits. If you wish to use those benefits, you may file a claim with your insurance company on your own. We will provide you a detailed receipt with our tax identification number, a diagnostic code, and a procedure code. Please keep in mind that our office will not be responsible if your insurance plan does not reimburse you for our fees.

At this time, only credit cards and cash are accepted as forms of payment. Please call the office 804-897-9355 for more information about services provided.

Medication Refills

Please note that medication refills require at least five business days notice. In order for us to serve you better we request that you call your pharmacy for refills and have the pharmacist fax us the request.

Please also note that in the event of a missed, rescheduled, or cancelled appointment, your medications may not be refilled. In order for you to continue medication, you will need to be seen in person at least every three months, unless your physician has suggested otherwise. Depending on your symptoms, you might need to be seen more frequently.

Medications that are controlled substances and require original handwritten prescription will be written at the office only during normal business hours, and with a five business day notice.

Refills via phone will be accepted only in the case of an emergency, which does not include missed appointments.
